KSU Ohio Sales Tax Exemption Certificate

This Ohio Blanket Sales Tax Exemption Form is provided for 17c��ҳ�� employees who need to provide a sales tax exemption to a vendor for purchases by 17c��ҳ��. Please review Sales Tax Exemption Information prior to completing the exemption certificate.

The Tax Identification number on the exemption form is 17c��ҳ��'s Ohio Sales Tax number and not the Federal Employer Identification Number. 17c��ҳ��'s Federal Employer Identification Number is found on IRS Form W-9.

If you are a vendor that requires a sales tax exemption certificate, please contact the 17c��ҳ�� department that is making the purchase to obtain a signed form.

Instructions:

Please add the Vendor Name, Department address, sign and date the form. Anyone authorized to make purchases with 17c��ҳ�� funds may sign the form.
